Metabolic liver disease characterized by ≥5% hepatic steatosis not due to significant alcohol use; encompasses NAFL (simple steatosis without inflammation) and NASH (steatohepatitis with inflammation and cell injury ± fibrosis). Recently, NAFLD was renamed MASLD with NASH termed MASH to emphasize underlying metabolic risk factors.
Extremely common (affecting ~25% of adults) and now the leading cause of chronic liver disease. Usually tied to obesity and type 2 diabetes (hepatic manifestation of metabolic syndrome). Can silently progress to cirrhosis and is an increasing cause of hepatocellular carcinoma and liver transplantation.
Often asymptomatic; usually identified on routine labs (mild ↑ALT > AST) or imaging (ultrasound showing a bright fatty liver). Vague fatigue or right upper quadrant discomfort can occur; hepatomegaly may be present due to fat infiltration.
Most patients have features of metabolic syndrome: central obesity, insulin resistance (type 2 DM), hypertension, and dyslipidemia. NAFLD can also occur in non-obese patients ("lean NAFLD") but is less common.
Advanced disease (fibrosis progressing to cirrhosis) may present with signs of chronic liver disease (eg, splenomegaly, spider angiomas, palmar erythema) or complications like ascites, but this typically occurs after years of unrecognized NASH.
Suspect NAFLD in patients with metabolic risk factors and unexplained mild elevation in liver enzymes (ALT usually > AST, but AST/ALT <1). Exclude other causes: review alcohol intake, test for viral hepatitis (HBV, HCV), autoimmune markers, iron studies (hemochromatosis), etc.
Imaging: ultrasound often shows increased echogenicity ("bright" liver). MRI-PDFF (proton density fat fraction) can quantify hepatic fat. FibroScan (transient elastography) measures liver stiffness to noninvasively assess fibrosis – useful to stratify risk without biopsy.
Use noninvasive scores to estimate fibrosis: e.g., FIB-4 index (uses age, AST, ALT, platelets) or NAFLD fibrosis score. A low score can reassure (low fibrosis risk), while a high score suggests advanced fibrosis (indication for further workup or biopsy).
Liver biopsy is the gold standard to differentiate simple steatosis vs NASH and to stage fibrosis, but is not done in everyone – reserve for uncertain cases or if it might change management (e.g., before starting certain therapies). Histology of NASH shows macrovesicular steatosis, hepatocyte ballooning degeneration, Mallory-Denk bodies, and perisinusoidal ("chicken-wire") fibrosis in zone 3.
Monitor labs and imaging over time. If fibrosis is present, patients need surveillance for progression. Once cirrhosis is established, manage as advanced liver disease (portal hypertension screening, HCC surveillance every 6 months).

First-line: lifestyle modification with diet and exercise. Target weight loss of ≥7–10% of body weight (gradual weight loss; avoid crash diets). Diets like the Mediterranean diet (low saturated fats, low fructose) are recommended. Even modest weight loss improves steatosis and insulin sensitivity.
Manage components of metabolic syndrome: optimize glycemic control in diabetes (metformin, etc.), treat dyslipidemia (statins can be safely used in NAFLD to reduce cardiovascular risk), and control hypertension. Avoid alcohol excess and hepatotoxic drugs (like unnecessary acetaminophen or herbal supplements).
Pharmacotherapy (no definitive FDA-approved drug yet, but some used off-label in NASH): Vitamin E (antioxidant) can be considered in non-diabetic patients with biopsy-proven NASH; pioglitazone (TZD insulin sensitizer) is often used, especially if the patient has T2DM; and GLP-1 agonists (e.g., liraglutide) or SGLT2 inhibitors have shown improvement in NASH in trials. In severe obesity, bariatric surgery is an option and often leads to NAFLD improvement. Advanced cirrhosis may ultimately require liver transplant.
In NAFLD, ALT is usually higher than AST (unlike alcoholic liver disease). AST and ALT are typically only mildly elevated (often <5× upper limit of normal). An AST:ALT ratio > 1 in a "nonalcoholic" patient may signal advanced fibrosis/cirrhosis.
"Cryptogenic" cirrhosis: many cases of unexplained cirrhosis (especially in obese diabetics) are actually due to burned-out NASH. These patients still require routine cirrhosis care (screening for varices and HCC).
Weight loss can reverse simple steatosis and even improve NASH. A ≥10% reduction in body weight can significantly reduce liver fat and inflammation, and may even regress fibrosis if achieved early.
Beware other causes of fatty liver: e.g., certain drugs (methotrexate, amiodarone), rapid weight loss or malnutrition, and rare disorders can also cause hepatic steatosis – these must be ruled out before labeling it NAFLD.
Signs of advanced fibrosis/cirrhosis in NAFLD (e.g., low platelets, spider nevi, ascites) – indicates progression to end-stage liver disease. At this stage, initiate HCC surveillance and manage portal hypertension (β-blockers, endoscopy for varices).
Unexplained rapid worsening of liver enzymes or liver function in a NAFLD patient – consider a superimposed injury (acute viral hepatitis, drug-induced liver injury) or development of HCC. NASH cirrhosis patients have a risk for hepatocellular carcinoma, even if liver enzymes normalize.
Lean NAFLD (steatosis in a non-obese patient) – warrants careful evaluation for secondary causes (viral hepatitis, alcohol use, medications, etc.) or underlying genetic predispositions. Lean patients can still have metabolic risk factors (like visceral fat) despite normal BMI.
Patient with obesity or metabolic syndrome + incidental elevated LFTs or fatty liver on imaging → suspect NAFLD.
Exclude other causes: take alcohol history, test for viral hepatitis, iron studies, autoimmune markers. If none explain the findings, diagnose NAFLD (by exclusion of others).
Estimate fibrosis risk: calculate FIB-4 or NAFLD fibrosis score. If high or indeterminate, proceed with further assessment (e.g., FibroScan elastography or consider liver biopsy) to stage fibrosis.
All NAFLD patients: initiate weight loss plan (diet, exercise) and manage comorbidities (diabetes, hyperlipidemia, etc.). If NASH with significant fibrosis, discuss pharmacotherapy options (e.g., vitamin E, pioglitazone, GLP-1 agonist).
Surveillance: Monitor liver enzymes and metabolic parameters. If fibrosis stage F3 or cirrhosis (F4), screen for complications (varices, HCC) and consider referral to a hepatologist (for possible trials, transplant evaluation if decompensated).
Overweight patient with no history of alcohol presents with cirrhosis (e.g., ascites, variceal bleed) – workup is negative for hepatitis B/C and other causes → likely NAFLD (NASH) as the cause of cryptogenic cirrhosis.
