Chronic inflammatory arterial disease marked by cholesterol-rich plaque (atheroma) buildup in medium and large arteries, leading to progressive lumen narrowing and reduced blood flow.
Atherosclerosis is the #1 killer worldwide, underlying most heart attacks and ischemic strokes. It also causes peripheral arterial disease (limb ischemia) and is a common cause of renal artery stenosis leading to secondary hypertension and chronic kidney disease. Nearly half of all deaths in Western countries are attributed to atherosclerotic cardiovascular disease.
Often asymptomatic for decades until significant artery narrowing or plaque rupture occurs. Fatty streaks (early lipid lesions) can appear in childhood, but clinical symptoms typically manifest in middle-aged or older adults with risk factors (e.g. smoking, diabetes, hyperlipidemia, hypertension).
Manifestations depend on the affected vascular bed: coronary arteries → angina pectoris or myocardial infarction; carotid/cerebral arteries → TIA or ischemic stroke; peripheral arteries (legs) → intermittent claudication (exertional calf pain); renal arteries → refractory hypertension and ischemic nephropathy.
Clues on exam include arterial bruits (e.g. carotid bruit in neck, abdominal bruit in renal artery stenosis), diminished peripheral pulses (in PAD), and signs of chronic poor circulation (cool extremities, hair loss on legs). Patients often have coexisting risk factor findings (e.g. xanthelasma from hyperlipidemia).
Screen for risk factors in at-risk patients: check lipid profile, blood pressure, glycemic status, etc. and encourage aggressive risk factor modification if abnormal.
Use appropriate tests to detect atherosclerosis in specific beds: e.g., an ankle–brachial index (ABI) test for PAD, carotid ultrasound if a carotid bruit is present, and a stress test (exercise ECG or imaging) for suspected coronary disease.
Confirm and quantify disease with imaging when needed: a positive stress test warrants coronary angiography for definitive evaluation and possible intervention; significant carotid stenosis on ultrasound can be confirmed with angiography. Imaging (CT angiography or calcium scoring) can also assess plaque burden noninvasively.
Remember that atherosclerosis is systemic: finding it in one artery (like leg arteries in PAD) should prompt evaluation and management of other vascular territories (heart, brain) for coexistent disease.

Aggressive risk factor control is key: lifestyle changes (heart-healthy diet, regular exercise, smoking cessation) and medical management of hypertension, diabetes, and dyslipidemia. High-intensity statin therapy is indicated to lower LDL and stabilize plaques; add ACE inhibitors/ARBs for BP control and other medications as needed to reach targets (e.g. HbA1c <7%, BP <130/80).
Antiplatelet therapy (e.g. daily low-dose aspirin) is recommended for secondary prevention in patients with established ASCVD to reduce thrombosis risk. Manage anginal symptoms with beta-blockers or nitrates as appropriate, and supervise exercise therapy for PAD to improve walking distance.
For advanced localized disease, consider revascularization: angioplasty and stenting (e.g. coronary stents for severe CAD, peripheral angioplasty for critical limb ischemia) or surgical bypass (e.g. CABG for multivessel CAD). High-grade carotid stenosis (>70% with symptoms) may warrant carotid endarterectomy to prevent stroke. In acute events (MI, stroke, limb ischemia), urgent interventions like thrombolysis or angioplasty restore perfusion.
Mnemonic: Response-to-injury – a classic model of atherogenesis: endothelial injury/dysfunction → inflammation and LDL entry → foam cells (fatty streak) → fibrous plaque → plaque rupture and thrombosis.
Plaque progression: fatty streaks (foam cell deposits) → fibrous plaque (smooth muscle migration & collagen cap) → vulnerable plaque with thin fibrous cap and large necrotic core (prone to rupture leading to clot formation).
Veins are generally spared: atherosclerosis affects arteries (high-pressure vessels). Veins do not develop atherosclerotic plaques unless transplanted into arterial circulation (e.g. vein grafts in bypass surgery).
Plaque rupture can trigger acute thrombosis: watch for sudden rest pain (unstable angina or myocardial infarction), acute neuro deficits (stroke), or an acutely cold, pulseless limb (acute limb ischemia – 6 P's). These emergencies require immediate intervention to restore blood flow.
Signs of critical limb ischemia (rest pain in feet, non-healing ulcers, gangrene) indicate end-stage PAD and impending limb loss — prompt vascular evaluation for possible revascularization or amputation is needed.
Exceptionally early-onset atherosclerosis (e.g. MI in a patient <40) suggests familial hypercholesterolemia (genetic very high LDL); look for xanthomas and treat aggressively.
Patient with risk factors (smoking, diabetes, high LDL, etc.) or symptoms suggestive of CAD → perform a stress test (exercise ECG or imaging) to screen for inducible ischemia.
If stress test is positive for ischemia → proceed to coronary angiography to delineate lesions and treat (stent or bypass if indicated). If stress test is negative, manage risk factors medically and investigate non-atherosclerotic causes of symptoms.
For TIA/Stroke workup → obtain carotid ultrasound; if severe carotid stenosis is found, consider revascularization (endarterectomy or stenting) in addition to medical therapy.
For claudication → measure ABI (ankle-brachial index); significant PAD (ABI <0.90) warrants exercise therapy, risk factor management, and if critical or lifestyle-limiting, arterial imaging and revascularization consideration.
